Tis the Season for Open Enrollment
For Members/Employers
Renewals for 2021 dropped in the mail Nov. 2! Inside the packet, you will find a hard-copy of your renewal that indicates your health care rates for 2021, along with instructions regarding the steps you need to take to renew your current plan or make changes for you and any lives covered, including employees (if applicable). You do not need to log in to an online enrollment system; however, your response regarding renewal is required during the open enrollment period, which runs through Dec. 31, 2020. If you have not received your packet in the mail, or if you wish to receive an electronic copy of your renewal, please contact us at 1-844-205-9579 or info@40Square.coop. Please note: Electronic copies of your renewal will only be sent by request.
As an employer, if you wish to continue your coverage with 40 Square, there are steps you need to take on behalf of any lives covered under your plan. To renew or make changes to your coverage, follow the Employer Renewal Submission & Disclaimers (“Checklist”) instructions included in your packet. If you have employees, regardless if they are on the health plan or not, you need to follow the checklist steps as well. For Employees
Your employer provides you coverage through the 40 Square Health Plan. Group renewals were mailed directly to your employer. Should you have changes you need to make or want to review your rate, please contact your employer directly to obtain that information.
New Aetna Partnership
By now should have received multiple communications via email and/or mail in regards to the new partnership with Aetna beginning January 1, 2021. All enrolled employees will receive a new ID Card in the mail in mid-to-late December. Be certain to use your new ID card when seeking care after the first of the year. Aetna brings many new plan resources and features to our 40 Square members. We encourage you to visit our Plan Participant page to check out many of these new features, and to find links to things such as finding a provider in the Aetna network.
